Start > run > ipconfig/all , post the screen shots.

Try changing to these values. Write down the previous one

You have a problem with the line most likely.

A simple testing at your end can be fruitful in deciding as to where the actual problem is ..........
though looking at your post it seems as if that the problem lies either with the line or with the modem ............
In order to make sure that your modem is working properly you may try your's friends modem if it's possible with the same modem type or as well with different modem type just to be sure about the modem .
the line problem can be verified by checking the respective SNR,attenuation values of your modem type which can be checked via opening the modem web-page in a browser.
sometimes such a problem may also occur due to wrongly fixed splitter ......... so do check that the main line first goes into the splitter then from the splitter one line goes to the telephone and the other to the modem .
