Can anybody please provide me the application format to get discount on a Broadband plan for senior citizens ?

I need a template .

I am not sure whether there is any discount for senior citizen. There is a discount for government and quasi-govt employees.

You have to apply in the exchange. They will give you a form to fill. Indicate on the form that you are a government employee.

Just4kix is right. There is a prescribed format available at your local exchange. You need to get it attested by section head at office.

is it available somewhere in the net ? I should get a print out of that and carry it to the local exchange then. If they don't find it then I could show them my printed copy and get the required things done .....my local exchange is small .
... I am sure they'll not be having any such prescribed format with them

So, if you kindly tell me if the format is available online somewhere OR if you tell me some similar kind format at least that would be very much helpful.

Not hopeful to find on net. I got it from the exchange. Sorry, don't remember the format.

There is no special printed form available.
Promotional Scheme of Broadband for Government Employees.
Will be effective from 1st of next month , if you apply now.

I have applied for the concession. They gave a form to fill up and get attested.

Download the form and print it. Fill it up and submit it at the exchange.

There is a facility for online submission also. Most people have reported that "IT DOES NOT WORK".

Completely "Out of Tune"
Nothing to do with Govt employees 20% concession, or
format for an application form TO AVAIL SAME.

Para two; May be I had misunderstood what @andromeda
actually wanted.
(a) Apply for a New Broadband connection
or
(b) avail 20% discount in BB rental/ usage as
applicable to Govt employees serving /retd etc.
and wanted a specimen form.

The BB application form is same for all people. Government employees have to submit another application for claiming the concession. There is no separate form for such category.
