Give a written application at your exchange stating the same. That will do. You can also talk to someone in the admin department for better results.

see post by @meetdilip. Once for all BB connection
disconnected. Your security deposit( if any) will
be credited in future bills as refund. ( mention this
    in your application
    2.
You can avoid paying present tariff plan
charges, every month, and reconnection charges ,
    and retain present userid/pw etc.
If modem on rent will be charged.

Change existing tariff plan to BBGZero.
( usually wef 1st of following month.
but may be effected within 2 or 3 days also.
Check in your Usage portal )

    2.
Change here. No need for log in. Note lead/ref number.

    3.
If you want Safe custody of telephone, then give application
for retention of telephone ( mention months required: max 6 months I think )
    low charges now.

    @lijorijo

    check all details here.
    Bharat Sanchar Nigam Ltd.
    Add to book mark.

You can surrender Post paid and use only prepaid plans.
or in addition to Post paid plans.
Condition: you must have a BSNL Landline telephone connection.
Prepaid ID/PW can be used from any BSNL L/L phone anywhere in India ( Like Sancharnet dialup account)
