i do port forwarding with bit comment and it worked , but it doesn't work with hlds,i dont know what to put in blank fields ! and what to type in custom USER DEFINED Field.

Try portforward.com. Select modem as Nokia Siemens C 2110 (similar interface) and then select the application you want to port forward. You will get the values and screen shot. Very easy.
