Can you access the modem menu pages ?
Give correct model number of the Cable modem.
How do you connect " Dialer" mode ? No Port forwarding required . All Ports open.
or
"Always On" ( userId/PW stored on wan page of modem )?

use DMZ host settings to open a port in your router/modem.
