As per the directive from the Department of Telecom (DOT), following Documents need to be submitted with the Prepaid Enrollment Form so as the company can give a SIM card :

For Identity/Address Proof

1. Passport
2. Arms License
3. Driving License
4. Ration Card with Photo & address
5. CGHS / ECHS Card
6. Certificate of address having photo, issued by MP / MLA / Group A Gazetted officer in letter head
7. Certificate of address with Photo from Govt. recognized educational institutions (for students only)
8. Address card with photo issued by dept of Posts, Govt of India
9. Photo ID Card having address
10. Caste and Domicile Certificate with Address and photo
11. Pensioner Card having Photo & address
12. Freedom Fighter card having Photo & address
13. Kisan Passbook having Photo & address

Identity Proof

1. Income Tax PAN
2. Photo Credit Card
3. Certificates of photo identity, issued by village panchyat head or equivalent authority (for rural areas)
4. Ration card with Photo
5. Smart card
6. Passbook of post office

Address proof

1. Ration card with address
2. Water Bill (not older than last 3 months)
3. Telephone Bill for Fixed line (not older than last 3 months)
4. Electricity Bill (not older than last 3 months)
5. Income Tax Assessment Order
6. Vehicle Registration Certificate
7. Registered Sale / Lease Agreement
8. Credit Card
9. Current passbook of post office
